How to Refer Church Members to the Center for Christian Counseling, Consultation and Training Inc. (The Center)

 

 

For Church Member who has health insurance that is covered at The Center:

 

• If church member has health insurance, have them call their insurance company and ask “If it is a medical necessity does my insurance plan cover services at The Center”?

 

• If church member has health insurance that is covered at The Center, have the church member call The Center directly to set up an appointment.

 

• The Center will then set up an appointment with the church member with the appropriate therapist that can best help them.

 

• The Center will then bill the church member’s insurance company directly.

 

 

For Church member whose insurance company is not covered at The Center, but has the financial means to pay for professional therapy:

 

• Have the church  member call The Center directly to set up an appointment

 

• The Center will then set up an appointment with the church member with the appropriate therapist that can best help them.

 

• The church member will pay directly for professional therapy.

 

 

For Church member whose insurance company is not covered at The Center and needs some financial assistance from the church for professional therapy:

 

Each church can contract for a set amount charged for their church members. If a contract is set, the Center is willing to give a grant for individual, couples and family sessions for each church member. The usual fee for an individual, couple or family therapy session is $140 and for a two hour group is $90.

 

• The Church will determine if they want to provide any financial assistance.

 

• If the church decides to provide some financial assistance, the authorized person at the church will fill out a Counseling Referral and Fee Agreement form (see separate form).  This form includes:

 

• Number of therapy sessions the church is willing to provide financial assistance.

  • The amount per session that the church will be responsible for and the amount the church member will be responsible.

  • Who will be responsible for the $30 one time initial session charge (Church or church member.)

  • Where to send the billing and to whose attention.

  • After the above information is completed, the church member will sign at the bottom of the form authorizing a release of billing information to the church.

  • The authorized church representative will also sign the form.

  • The church representative will fax the form directly to The Center at 608.274.8783 before the church member’s first appointment.

 

• Have the church member call The Center directly to set up an appointment.

 

• The Center will then set up an appointment with the church member with the appropriate therapist that can best help them.

 

• The Church member will pay their portion and the church will be sent a bill on their agreed upon portion.
